"Lesson learned": how ?

As an any regular reader of the FR forum can tell you, almost every FR contains the mention "LESSON LEARNED". The FR author writes what happened, realizes where he fucked up, and energically writes in capitals "LESSON LEARNED: I must approach", or  "LESSON LEARNED: I must isolate" , or "LESSON LEARNED: I mustn't make out with a chick who gave a BJ to a guy 1min prior to meeting me", you name it.

But is writing "LESSON LEARNED" enough to actually learn the lesson of what happened ? Far from it. You can see it in any journal here, the guy keeps repeating the same mistakes again and again. It struck me when I re-read my personal (not online) FRs: I was disturbed by the amount of lessons experience taught me from which I didn't learn anything. I kept on making the same mistakes again and again for weeks. 

If repetition is so necessary to any learning, how can anybody be sure that a lesson is learned as soon as any event or fuck up occurs ? It seems impossible. Therefore, it will be more useful to replace every "LESSON LEARNED" by "Here is the process by which I will actually learn my lesson..."

In a nustshell:

-Are you making sure you understand the lessons experience is teaching you ?
-Are you making sure you are actually learning them ?
-How are ensuring that the lessons are actually learned ?

The point of this post is going from "I hope I learnt my lessons" to "I effectively learned my lessons". You can say you learned when you don't make the mistake any more. 

What are you planning to do to make sure the lesson is learned?

For me it takes 3 times to fuck up  a certain element of game to realise, and have it etched on my brain, that I should do something different. 

After said 3 fuck-ups I am usually all good.
